Monitor Audio keeps cutting out when changing scenes

Please help, my video sources are set to monitor and output but when I switch scenes the monitor audio cuts out quite often. The meters still read audio and it still goes out to the stream fine. To fix I either have to change the audio settings monitors device to a different output and then switch back or go to the advanced audio properties of the source playing and change the monitoring to "Monitor Off" and then back to "Monitor and Output".

I sometimes hear a click or slight hiss when this happens. It happens when I output to my nvidia HDMI out or using my internal Realtek drivers. I notice this has come up on the Mac Forum as well. I have only just realised it does it when the source is on a different drive. I thought I'd found a temporary fix as when I changed the source of my media files to the same location it appeared to work. Having just reloaded and created a new show I am having the same problems even though all my media files are in the same directory. Aaaaarg.

I've tried reinstalling my sound drivers and this issue still happens. I've also reinstalled OBS but just the program files not user settings as I don't want to lose my current settings.

Please help. I run livestream service every Sunday and need this to function properly.

I'm still on OBS v25.0.8
since March, when I started using OBS, I noticed that it was not uncommon for our pre-recorded videos to 'glitch' the first time played using OBS's default Media Source [regardless of video format/source/resolution/bitrate, etc]. As such, my practice has to be 'pre-run' our stream and play (at least a portion) of EVERY pre-recorded video before the live stream [and videos are NOT set to 'Close file when inactive'].
This happened on multiple computers, all with more power (CPU, GPU, RAM, etc) than required to stream. And videos on SSD... so not sure why, but the pre-run prevents the problem. And even now, when setting up a love stream, or doing the pre-run, I still occasionally run into the audio 'glitch'. For me, it hasn't been worth it to dive deeper into the issue as my work-around suffices, and that time consuming, and is desirable for other reasons (basic QA)

This is still a problem as of now. Although, if you activate 'loop' the sound will come back at some point when it starts a new loop.
