Microphone Audio Monitoring Not Working (macOS)

I am using the latest stable version of OBS and the latest stable version of macOS Monterey on my 2020 27" Intel iMac. I want to use OBS for Twitch streaming games. I use the BlackHole program (2 channel) to capture desktop/game audio. When I record, my desktop audio and my mic are recorded fine and I can hear the desktop audio in my headphones. But I can not hear my microphone with Monitoring and Output on.

I am new to OBS so the solution is probably simple, but after some googling I can't find any threads addressing my exact problem on macOS and BlackHole.

My mic channel (from my Scarlett 2i2) AND my desktop channel both show working audio when I Monitor and Output from my mic, but I hear nothing.

So, here are my current settings:

In my Audio Midi Setup app I have a Multi-Output Device setup like this. This is the system-wide sound output device I would use when streaming, and what I have used for recording/testing.

The mixer acts like I should be able to hear my mic (this is just me talking with no other sounds).

Perhaps the most important part: these are the settings I must have to even see the desktop channel react to my mic. I tried to switch the monitoring device to my headphones, but then no sign of life in the desktop channel of the mixer and of course no sound in my headphones.
