Because a mp4 can contain a video encoded with

H.265
MotionJPEG
MPEG-1
MPEG-2
MPEG-4
MPEG-4 AVC
VP9
XviD
etc...

MP4 is container, inside you have what?
e.g. h.264 or other codec.
Use mediainfo on video file and past here.

I think, you have aggressive security software settings. Make sure that the whole OBS Studio folder is in the white list of your security software.
Also, you may try to run OBS Studio as administrator.
