Don't record directly to mp4. Use mkv or flv instead. mp4 requires very specific metadata to be written at the close of the file, so if anything happens that interrupts a clean ending, then you'll end up with a corrupted file.

If you need mp4 for editing purposes, you can use the remux feature under the File menu after the recording is done. It's just a container swap, no re-encoding is done there.

Okay I got the flv to work but the image is pretty blocky?

Any way to fix this? It was perfect fine in mp4

Is your recording was spontaneously interrupted? Is the crash-reports folder of the OBS Studio is empty?
