You have to switch to "Advanced mode" and set your desired value in the setting "Keyframe Interval"

Settings -> Output -> Output mode -> Advanced

I use https://inspector.twitch.tv/#// to check stream keyframe and it always 2 no matter what I put there. Only Xaymar FFmpeg encoder helps but not NVENC.

You're going to increase the chances of playback problems by going above 2 for keyframe interval.

Twitch requires a keyint of 2 in their stream ingest spec.
If you are following a 'best settings guide' telling you to use 4, stop, the author is braindead.
