Please help me understand what is happening with my audio here (video is fine).
In both scenarios, A & B, I can see the audio from the ATEM in OBS sources. If I record with OBS in case A, I only hear Headset mic via Mic/Aux. Case B, I see ATEM audio twice but it is on recording only once.
Why can I see the ATEM audio in my OBS audio sources but it is not recorded? Also, am I correct in assuming that what gets recorded is identical to what goes on my stream?

There are several audio handling concepts to consider:
streaming is always one audio track only
recording can record one audio track only or multiple audio tracks
in simple output mode (Settings->Output->Output mode), both streaming and recording will always stream+record one and the same track
in advanced output mode, you can define up to 6 different audio tracks to put audio into
in advanced output mode, which audio track goes out to stream can be defined in Settings->Output->Streaming->Audio Track (check one)
in advanced output mode, which audio tracks are written to the media file for recording can be defined in Settings->Output->Recording->Audio Track (check all tracks you need)
which audio source in OBS is routed to which audio track can be defined in Edit->Advanced Audio properties. This window is scene-specific. It includes all audio sources from Settings->Audio->Devices as well as all audio sources you might have added to the current scene. To the right, you see a matrix, where you can assign each source to each track you like.
In your case, you might miss one or two checks in that matrix for the audio track you're inspecting in your recording.
Also, keep in mind that any media player will output one track only even with multi-track audio. No media player will ever mix audio tracks. You can switch track within the player, but no player will output more than one track at a time. If you want to output a mixed version of your tracks, you need to this in some postprocessing software.
For easy preview of your audio, you can add one audio track that contains a mixed version of all audio sources and additional tracks with one source only like this:
In this case, track 1 contains a mix of all audio sources, and the additional tracks one source only. These can be used to create a customized mix in postprocessing. And track 1 could be the track you're using for streaming.
