Headphone monitoring woes.

Been using OBS for a long time. Had headphone monitoring working. Then it stopped working.

I then switched the monitoring output in settings to "Default" rather than the headphones. And it started working again.

The concern:
I'm pretty sure (but don't have logs to prove it) that in the past it was the other way around (ie: had monitoring on default, then it stopped working, then switched to headphones, and it started working again.) Would like to understand the issue so that I can prevent it happening in the future.

Any insights appreciated.

I'm guessing this is the issue:

[audio monitoring: 'Qmix'] audio_monitor_init_wasapi: Failed to activate device: 88890004

The Google is not turning up anything useful other than "some other app is using your device." Only things running are Zoom, OBS and Firefox. Which are the apps that have always been running.

Any insights?
