Question / Help "Graphics Card Error" Every Few Startups + Game Cap Randomly Black Screens/Glitches Out Between Sessions

*I don't have a log for this yet, since everthing has been running smoothly for the last few days*
This started happening two weeks ago. The first issue is that once every third startup or so, OBS will hit with me an error that states that my graphics card isn't detected. A simple restart of the program solves this for a few days.
Problem #2:
I normally leave OBS and the Xbox App open between sessions to save myself a little time when I get ready for the next session. Here lately, whenever I come back for another sessions, I'll get the Xbox stream going in the Xbox App, but when I tab over to OBS to check everything, the Game Cap is black.
No biggie there. I remove the source and re-add it like normal. But sometimes it still doesn't pick up the stream, even after I tab over to the Xbox App. SOMETIMES, instead of being my full-sized Game Capture window, it'll suddenly be scaled down to maybe 1/16 of it's usual size AND, instead of showing a frame from the Xbox stream or a black screen, it will show a completely random in-game screenshot from other games. Usually it's some random frame from an Apex match, but what gets me is that it's never a shot from a game I was involved in. It's always some random players. I don't get it.
Anyways, a few back-to-back restarts of OBS and the Xbox App has become my temporary fix for now. But I'd really like to find out what's causing all of this and what can be done to fix it for good. Any and all assistance would be greatly appreciated.
