This seems to be happening to a lot of people recently including myself, was speaking to a moderator for hours yesterday and still no fix however some people have said that right click game capture, click properties and disable multi media compatibility (if it is checked)

Code:
12:48:54.647: Output 'adv_file_output': Number of lagged frames due to rendering lag/stalls: 1456 (28.3%)
Lookls like GPU overload. Try again with reduced GPU load (fps limit/vsync to 60fps and lower game details).

Not going to lie, I think this might be because of the new NVIDIA drivers, which is causing everything to be so fucky, there have been so many reports of the same issue, in the last day or so (including myself)

I am going to rollback the driver and see if that helps at all

Driver rollback won't help, only rollback to Win10 anniversary build might help. The Creator Update introduced those problems.
At least I can play smooth on creator update, but my 60fps recordings are suffering some microstutter, although not frames were skipped or lagged.

Same issue here with choppy quality. The weird thing is that sometimes a settings combination in OBS works and sometimes it does not, making the stream stop and go all the time like having 1 fps. If it works then it is choppy/fuzzy. Very pixelated. Rescaling and lowering FPS does not help either. It is not the first time i tried streaming. I think it was around 5 years ago where i tried to stream with the same hardware and the output looked smoothe with 60fps and clean 1080p even with fast movement. Either OBS or the GPU drivers updates have caused this problem.
