I'm just reporting this for informational purposes. Not sure if this is something that can be fixed, but at least the OBS team will know, and anyone having the same problem as me will know how to fix it. I'm using Windows 7 Home.
In short, lots of games were not game capturing. Witcher 3 showed a black screen. CS:GO would capture the first "Hidden Path" title page, then get stuck there. Dying Light worked perfectly. I could go Borderless Window/Monitor Capture, but I didn't want to rely on that in case I wanted to stream a game that didn't support that feature.
Problem was RivaTuner Statistics Server which was installed along with MSI Afterburner. I'm not OCed right now, but I like to use MSI for monitoring. Since I have Witcher 3 through GoG, I needed a way to monitor FPS, so I used MSI. Apparently, it works alongside RivaTuner, which hooks the game as well, which is what was causing the issue.
At first, I couldn't uninstall RivaTuner, so I closed out MSI, then tried to uninstall. It uninstalled, rebooted, and now everything works except that I can't monitor FPS through MSI anymore. I can still monitor everything else like GPU Usage, Temperature, CPU, etc. It was only the FPS monitor that was causing a conflict.
Also, this issue didn't present itself right away, which is why it was hard to identify. I was able to game capture perfectly for a few days before it started becoming more and more unreliable.
TLDR: If game capture isn't working, try disabling RivaTuner or any other program that uses a hook to show FPS. Or any program that hooks for any reason, for that matter.
