Try this:
As you scale your canvas in VIDEO settings, do not scale in output settings. And use Lanczos to get less pixelation.

Output settings: use NVENC (encode with your graphics and release load from your CPU)

Run OBS 64 bits.

the log showing

16:24:26.838: Output 'adv_stream': Number of skipped frames due to encoding lag: 7431 (10.5%)
16:24:26.838: Output 'adv_stream': Number of lagged frames due to rendering lag/stalls: 785 (1.1%)

Try dropping the cpu preset to ultra fast as that is potentially causing the lag.

presuming that 4690K isn't OC then you could look at this to get more cpu performance out of it helping with the x264 software encoding.

Try increasing OBS priority (maybe in OBS advanced settings) or while in game, alt+tab and use task manager details tab, reduce game's priority. Changes made to priority through task manager will need to be done on every game start/restart.

Also check for CPU overheating slowdowns. In task manager CPU usage, if GHz drops while over 50% then CPU might be doing overheating slowdowns. It is normal to drop GHz at less then 5% CPU usage to save power.
