Duplication of 720p source

I'm reaching out for some assistance with a problem I'm experiencing while streaming via OBS, it seems to be beyond my skill set.

While setting up OBS, I've noticed that the output signal has 720P listed twice. I'm not quite sure what it means or how to fix it. Has anyone encountered this issue before?

There's another problem as well. When viewers choose to watch my stream in low quality, the audio starts to "lag". This is extremely peculiar as I have not faced such issues with this device before.

It's not clear what configuration of what app or service you show. The OBS Studio configuration doesn't look like this, it has a drop down list for the resolution and is accessed via Settings > Video.
