Duplicate Elgato audio in desktop track

It seems like every other day I'm having issues with OBS. For some reason, the audio from the Elgato track is going to the desktop track and I have no idea. I have uninstall and reinstall but nothing has changed. As far I can tell, I'm not hearing duplicates of the sound but stream will when I go live if I leave the desktop unmuted. When the Elgato is monitor off, it doesn't show up, only when the below is active.

As far as I can tell there is nothing in my settings that should cause this.

I'm so lost in trying to figure this out. Not sure if it will help but my log is attached as well.

Watch on first picture, you have Elgato selected to Monitor and Output, on second picture capture speakers.
You have loop, you record sound from elgato and record the same sound from speakers.
Disable global capture sound.

How do I do this? These are the only options I see for the channel
[mono, stereo, 2.1, 4.0, 4.1, 5.1, 7.1]

Global Audio Device, position Desktop Audio Device.
You play sound from Elgato (you select in Advanced Audio Properties, Monitor And Output).
In this configuration you play sound from Elgato on speakers and send sound to stream and send sound from speakers to stream. You have double sound from Elgato.
