Double Image

I'm new to OBS. I've been using XSplit for years but it's become so buggy for me that I have to switch. Here's my situation:

I conduct interviews over Skype and sometimes Zoom.
I capture my side of the video with a Canon VIXIA HF G21 camcorder feeding data into my PC via HDMI and a Blackmagic Intensity Pro card.
This arrangement worked fine in XSplit by adding something called Decklink Video Capture into a scene.
I can't get that to work in OBS but I do get a picture by choosing Blackmagic WDM Capture as my device. See attached screenshots.
My problem is that I'm getting a double image (see screenshot) - the same thing one above the other squished vertically. I have no idea why this is happening. Any ideas?
