Can't get all three mics to record

While recording all three mics simultaneously, we all can hear each other on our headphones, and the levels are showing activity like normal. Then when played back, each time, the video consistently shows one of the mics didn't get recorded, always the same mic. Very frustrating. Troubleshooting shows the issue isn't the mic itself nor the cable. On Pro Tools, all three mics get recorded. Only on OBS is everything seemingly normal until the vid plays back, and that one mic wasn't actually recorded.

Normally in audio software is the rule: "What you hear is what you get". OBS has another one: "What you hear is not always what you get"
You will have to explain better your setup because the cause can be a lot of issues...

In the process of simultaneously recording all three mics, our headphones all show activity, and we hear each other. In the video, when played back, it always shows the same microphone didn't get recorded. An extremely frustrating experience. The issue is not with the mic or the cable itself, according to troubleshooting. All three microphones are recorded using Pro Tools. The only time everything seems normal with OBS is when you playback the video and realize it didn't actually record the mic.
