Question / Help black screen using SIIG HD Video Capture Device

Hello. Brand new to OSB, so learning. I'm using a SIIG HD Video Capture Device. I set up a scene. OSB recognizes my device as a video capture device - a Fujitsu H5x Video Capture. However, I don't get my video, just a black screen. A log is attached. Any help is appreciated.

I'm encountering the same issue with a similar device. Any tips on this, I think it's a computer hardware issue but it's hard to figure out.
