Question / Help Black screen on Blackmagic Mini Decklink Card

I have set the input in the Blackmagic Desktop Software and am receiving video in the Blackmagic Media software. I then open OBS and am not getting anything.

I have followed a few other suggestions by Jim on a few other threads but am still yet able to resolve it. Your help would be much appreciated.

I am running a i5 3.5ghz Windows 8.1 machine with a EVGA GT 740 SC and 16GB of Ram. It has a Black Magic Decklink Mini.
