Question / Help Black magic Ultrastudio Express

I am having trouble getting the BMD Ulstrastudio Express to work. Searching the forums tells me that people had had apparent success with the Ulstrastudio Mini.

I am able to see the video in the Blackmagic Media Express app. I have set the settings in OBS to match -- 1080P29.97.

Attached is a screen shot of what I am getting.

For what it's worth, the Livestream Producer app works fine with the hardware setup; however, my goal is to use OBS for Facebook Live streaming.
