Question / Help Avermedia live gamer HD / PS4 capture

I just bought Avermedia Live gamer HD lite capture card so i can stream my PS4 gameplay in higher quality. It was super easy to set up, it has full support for OBS and getting OBS to output game audio to my headset was also easy.
And this is where the fun stops.
If i make OBS output audio to wave out or direct sound, i can hear the console audio, but no audio is being output to the stream. How to get both?
Second problem is that sides of my captured console are cut of. It looks like i'm trying to capture 4:3 format. My console is set to output 1080P signal. And base resolution in OBS is set to 1080P.
My desktop capture looks just fine. No cut sides of the screen. Just when i try capturing PS4.
Any idea how to solve these 2 problems?

Fast update. OK i solved the console capture display sides getting cut off by simply changing the audio output from waveout to directsound and back. For XY reasons this fixed the display format.
Now i only need your help with getting the audio to my headset and out to stream at the same time.
I am using Kingston hyper X cloud II. And built in mic for commentary.

Another fast update. I just added desktop audio capture to console capture scene. All problems solved. Thanks for the help. XD
