There is a sticky at the top of this forum about recording desktop audio on Mac.

It is slightly out of date, in that you will need this version of SoundFlower on versions of MacOS from El Capitan and above:

Basically you make Soundflower your default audio output device, set that as your audio source in OBS, and then either use OBS' monitoring feature to listen to the audio (preferably on headphones) or another program to forward the audio to a listening device, such as the free app LineIn:
