I also have strange things going on with my 1950x Threadripper during streaming. I am hoping its a software issue. OBS maybe doesnt know what to do with high core count CPU's I have an issue where I drop 1 or 2 frames sometimes more at random times. No Idea Why. Check My Post Out If You Would Like.. Hope You Solve Your Problem.

Your log doesn't show any streaming session.

You need to start streaming until you see this problem happening, stop streaming, then get the log and upload it.

Hi,

1. What is your base resolution?
2. If your base resolution is more than 1080p: do the rescaling in the video tab, not the output tab
3. Is "slow" more lagging than "medium" or are both lagging the same?
4. I personally don't like the "Threads" option. Have you tried medium/slow without this option?
5. What encoder do you use in your elgato settings? GPU or CPU? If CPU --> change to GPU.

1.1080P
2.YES ALL SETTING 1080P
3. slow or medium both same crazy lag...
4.i have try without Threads same issue.
5.i don't think 4K60 Pro has encoder......

If you have a base resolution of 1080 then do not enable "Rescale Output" in your output tab. Also try disabling "Enforce streaming service encoder settings".

Here is the encoder option I mean in the Elgato 4k Capture Utility: click

Are you recording simultaneously via x264? If yes: try streaming only - without recording.
And my last suggestion: try using a higher process priority: click

Have you thought about running some benchmark tests like cinebenchR15? Maybe there is something wrong with your CPU / Motherboard? Your score should be around 3000 points.

Kind regards
Maggi
